Hawaiʻi State Law requires all students to meet physical examination, immunization, and tuberculosis clearance requirements before attending a childcare facility, preschool, or public or private school in the state. We work in partnership with the Department of Health to ensure that students meet the key health requirements.

신체 검사

다음 중 1년 이내에 완료해야 함:

• 하와이주에 있는 유치원이나 학교에 처음 입학한 날짜
• 중학교 7학년 첫 등교일.

예방접종

• 필수 예방 접종 (PDF) 학생의 나이 및/또는 학년에 따라 달라집니다.
• Children may be exempt from immunization requirements for medical or religious reasons, if the appropriate documentation is presented to the school. Religious exemption forms may be completed at your child’s school. Medical exemptions must be obtained from your child’s healthcare provider. No other exemptions are allowed by the state.

약물

자녀에게 응급 구조 약물이나 기타 매일/일상적으로 처방되는 약물이 필요한 경우 다음을 작성하십시오. 학교 건강 양식 학교에 제출하세요.

필수 예방 접종이나 신체 검사를 모두 완료하지 않은 학생은 다가올 진료 예약 확인서를 제출하면 임시 입국이 허가될 수 있습니다.

불안정한 주거에 있는 어린이들

자녀가 불안정한 주거 환경(McKinney-Vento법)에 처해 있거나 불법 이민자, 난민이거나 망명을 신청하는 경우(Plyer v. Doe에서 확인) 학교에서 건강 기록 이전을 준비하거나 커뮤니티 노숙자 문제 연락 담당자 학생이 건강 요건을 충족하도록 도와줍니다.
