Hawaiʻi State Law requires all students to meet physical examination, immunization, and tuberculosis clearance requirements before attending a childcare facility, preschool, or public or private school in the state. We work in partnership with the Department of Health to ensure that students meet the key health requirements.

身體檢查

必須在一年內完成：

• 首次在夏威夷的幼兒園或學校就讀的日期以及
• 首次進入七年級的日期。

免疫接種

• 必需免疫接種 (PDF) 取決於學生的年齡和/或年級。
• Children may be exempt from immunization requirements for medical or religious reasons, if the appropriate documentation is presented to the school. Religious exemption forms may be completed at your child’s school. Medical exemptions must be obtained from your child’s healthcare provider. No other exemptions are allowed by the state.

藥物

如果您的孩子需要緊急救援藥物或其他日常/常規處方藥，請填寫 學校健康表 並提交給學校。

未提供所有必需免疫接種證明或體檢證明的學生，在提供即將到來的醫療預約證明後，可獲準臨時入境。

住房不穩定的兒童

如果您的孩子住房不穩定（根據《麥金尼-文託法案》）或屬於無證移民、難民或尋求庇護（根據 Plyer v. Doe 案），他們可以在學校安排轉移健康記錄或 社區無家可歸者問題聯絡 幫助學生滿足健康要求。
