The survey information is used to improve parental involvement in the special education process and to increase outcomes for children with disabilities.

• The survey has 11 easy-to-understand questions and takes less than 10 minutes.
• Parents should complete one survey for each child receiving special education services.
• All responses are anonymous and not linked to you or your child.
• Schools provide the survey after each initial or annual IEP meeting.
• A third party processes all responses to ensure objectivity.
• The survey is available from July 1, 2025 to June 30, 2026.
