The Department invites parents who have a child with an Individualized Education Program (IEP) to participate in a short survey to see how well your child’s school is partnering with you and promoting involvement in your child’s education.

The survey information is used to improve parental involvement in the special education process and to increase outcomes for children with disabilities. It is important to note that your responses are recorded anonymously and are not linked to your child.

• 保護者の関与に関するアンケートは複数選択式で、回答には 10 分もかかりません。 
• 回答は匿名であり、あなたやあなたのお子様に関連付けられることはありません。
• 特別教育および関連サービスを受けているお子様 1 人につき、学年ごとに 1 つのアンケートに回答する機会を提供します。
• 初回または年次の IEP 会議の後に、お子様の公立学校からアンケートが提供されます。
• Ensures objectivity by being processed by a third party. Is available to be completed from July 1, 2025 to June 30, 2026.
