The Department invites parents who have a child with an Individualized Education Program (IEP) to participate in a short survey to see how well your child’s school is partnering with you and promoting involvement in your child’s education.
The survey information is used to improve parental involvement in the special education process and to increase outcomes for children with disabilities.
TUNGKOL SA SURVEY
- The survey has 11 easy-to-understand questions and takes less than 10 minutes.
- Parents should complete one survey for each child receiving special education services.
- All responses are anonymous and not linked to you or your child.
- Schools provide the survey after each initial or annual IEP meeting.
- A third party processes all responses to ensure objectivity.
- The survey is available from July 1, 2025 to June 30, 2026.

Printed survey samples are available in various languages to reference when completing the English survey version. Please contact your child’s school for an English survey with an accompanying postage-paid envelope.
