Effective transition planning and services are critical to success in education, training, integrated employment and independent living or community engagement. Multiple factors contribute to a successful life after high school.

Information has been included that will help families, caregivers, school counselors and service providers by offering guidance as you develop your academic or employment goals. It can also assist in identifying appropriate agencies, institutions, and programs that may best match your needs and interests as you plan your transition.

Ang Special Parent Information Network (SPIN) ay isang parent-to-parent na organisasyon sa Hawai'i na nagbibigay ng impormasyon, suporta at referral sa mga magulang ng mga bata at young adult na may mga kapansanan at sa mga propesyonal na naglilingkod sa kanila.
