Post-Secondary Transition

Effective transition planning and services are critical to success in education, training, integrated employment and independent living or community engagement. Multiple factors contribute to a successful life after high school.

Transition services helps to facilitate students with disabilities moving from school to post-school activities. In the state of Hawaiʻi, this process involves students with disabilities, families, educators, community service providers, Vocational Rehabilitation (VR), the Department of Health, counselors and employers, who are critical to a student’s life. 

This Family Engagement Transition Toolkit (PDF) is designed especially for emerging young adults with disabilities to utilize as a resource to support the course of transition planning for post-secondary education and employment. Information has been included that will help families, caregivers, school counselors and service providers by offering guidance as you develop your academic or employment goals. It can also assist in identifying appropriate agencies, institutions, and programs that may best match your needs and interests as you plan your transition.

The Special Parent Information Network (SPIN) is a parent-to-parent organization in Hawai‘i that provides information, support and referral to parents of children and young adults with disabilities and the professionals who serve them.
