Parent Involvement Survey

The HIDOE is surveying parents who have a child with a disability and an Individualized Education Program (IEP) to determine how well your child’s school is partnering with you and promoting involvement in your child’s education.  Public schools provide you with the opportunity to participate in a short survey, which will help the Department collect data for program improvement and reporting to the U.S. Department of Education, Office of Special Education.

Additionally, this information helps identify training needs related to the special education process. The results are used to improve parental involvement in the special education process toward improving outcomes for your child with disabilities. It is important to note that your responses are recorded anonymously and cannot be personally linked to your child.

ABOUT THE SURVEY

The survey consists of 11 items in family-friendly language. Parents with more than one child receiving special education services should submit one survey for each child.

  • The Parent Involvement Survey is multiple-choice and takes less than 10 minutes to complete. 
  • Responses are anonymous and not linked to you or your child.
  • Provides you with the opportunity to complete one survey per child receiving special education and related services per school year.
  • A survey is offered by your child’s public school after the initial or annual IEP meeting.
  • Ensures objectivity by being processed by a third party. Is available to be completed from July 1, 2024 to June 30, 2025.

Printed survey samples are available to download. Please contact your child’s school for a survey with an accompanying postage-paid envelope.
